How to...underline with sub?

I was told that it's quite possible in word to have something like there is on hard copy contracts.

Example of thing is something like below here. Except the YOUR NAME HERE part would be superscripted to the bottom of that line.
How can I do this, and make it so that when typed; the line and the YOUR NAME HERE part still stays there?

VBA would of course be acceptable, as well as most any other automated way to do this.
